Q: How do I, as a healthcare provider, make a referral to St. Luke Home Health Care, Inc.?
A: All referrals should be directed to our Intake Department; we are open Monday through Friday, 9:00 AM to 5:30 PM at (818) 956-9878 or feel free to fax your referral to (818) 956-9872. If you have a referral on a weekend, holiday, or after 5:30 PM on a weekday please call our on-call staff at the same number, (818) 956-9878. When referring a patient, please have the patient’s history, physical (if being discharged from a hospital or facility), insurance, and contact information available.

Q: Who pays for Home Health Care?
A: Most commonly, Medicare will pay for home care. However, there is certain criteria which must be met. Alternatively, Self-Pay or Private Pay is accepted, where you pay out of pocket all expenses associated with the care of yourself or your loved one. Often, third party administrators or private insurance will cover the costs associated with home care. Remember, home health criteria will vary with payors and eligibility must first be met and cleared before St. Luke Home Health Care, Inc.’s home health services can be initiated.

Q: How do I choose a home care provider?
A: Finding the best home care provider to meet patient’s individual needs deserves a little research, but it is time well spent. The quality of care and competency of personnel are areas of primary concern. Here are some questions to consider when deciding which home care provider would be best:

What services does the agency offer?
How long has the agency been serving the community?
Does your physician know the reputation of the provider?
Is the provider certified by the Department of Health?
What credentials does the provider maintain?
Does the provider employ insured, bonded and licensed skilled Nurses and ensure that it’s workers are functioning under written personnel policies and clinical protocols?
Is the agency a member of a state and/or national home care organization?  Remember, accreditation is an indication that the agency is held to a higher standard in the home health care industry.
